An Important Message from Three Village Behavioral Medicine

In consultation with my doctors and my family, I have recognized that my worsening and persistent disability related to Long COVID has made it impossible for me to continue my work with our patients and their families. As of today, I will no longer be able to provide any medical care or advice.

To mitigate the difficulties that my abrupt departure from practice will cause you, we plan to take the following steps:

  • Provide renewals for existing prescriptions at their current doses upon request until December 28, 2023, to allow you time to put new care in place without disruption of access to current medicines. Prescriptions will be sent Mondays through Thursdays for requests received by noon that day or carried over from prior days. Please plan ahead.
  • Providing records upon request will help with a smooth start with each patient’s next clinician. Records can be transferred directly to a clinician or can be sent to you.
  • Provide information about available treatment resources in the community. You will receive an email containing Referral Resources. Please contact us if you do not receive it in the next day.
  • Issue refunds in the near future where appropriate.
